Interesting question. I think the concept of the character was always destined to turn up, given the failed pilot. But it failed for reasons, none of which the showrunners were willing to let go of, only delay introduction. I honestly think she is an awful writing decision that shouldn't have happened, and has been atrociously handled. Abruptly introducing a new character into the heart of the cast badly unbalanced the structure.
Without her - the Bernadette/Howard storylines would have been accelerated, but I think we'd have had exactly the same lame, tired L/P mess. Sheldon would have just been peculiar in his own individual way, not forced into such a regressive childlike state. I hope. Though maybe you'd have had the same scatalogical humour and even more random drunken trouser-dropping.
